The core pathology involves mesangial expansion, glomerular basement membrane thickening, and nodular glomerulosclerosis (Kimmelstiel-Wilson lesions) due to chronic hyperglycemia-induced metabolic and hemodynamic changes. These changes lead to increased glomerular permeability and proteinuria, which is a hallmark clinical feature. The condition is a leading cause of end-stage renal disease (ESRD) worldwide. Pathogenesis includes advanced glycation end products, activation of the renin-angiotensin-aldosterone system, and oxidative stress. Clinically, it manifests as persistent albuminuria, declining glomerular filtration rate, and hypertension.
