The core pathology involves dysfunction in the dopaminergic pathways and iron metabolism within the central nervous system, particularly affecting the substantia nigra and spinal cord circuits. RLS can be idiopathic or secondary to conditions like iron deficiency anemia, chronic kidney disease, or pregnancy.
